- Strengthen infrastructure and referral pathways so families reach the right care quickly
- Improve quality and safety through shared data and proven patient safety practices
- Deliver whole-person care, with screening and follow-up for mental health, substance use and social needs
- Advance birth equity through culturally responsive, community-centered care

Cross-Sector Partners, One Shared Goal
The Perinatal Wellness Taskforce is the cross-sector action body of California's Perinatal Hub. Health plans, hospitals, public agencies, home-visiting programs, community-based organizations, cultural brokers and lived-experience leaders meet monthly to align local priorities, share data, identify gaps in care and coordinate solutions that improve how families move through the perinatal care system. Our work focuses on data-driven decision making, community alignment through policy, training and education, and workforce development.
